Appointment Coordination

We handle the scheduling of routine visits with primary care doctors, audiologists, dentists, specialists (e.g., cardiologists) and preventative screenings (e.g., mammograms, ultrasounds) along with the rescheduling and confirmation of all appointments and transportation, ensuring no details are missed.

Appointment Support Leader

Our trained, compassionate team accompanies elders from their place of residence to appointments, providing reliable transportation, mobility assistance, and communication support.

Communication with Family and Care Team

After each appointment we provide the family a detailed summary and follow-up care as needed, ensuring a smooth transition and continuity of care.

Professional Elder Care Consultant Team

All services are closely monitored by the Professional Elder Care Consultant Team, ensuring high standards of care and seamless service delivery.

Scheduled Appointments

The duration of each visit is billed on an hourly basis. Any additional charges, such as mileage at $0.70 per kilometer and parking fees, will be applied as agreed upon during the initial consultation.

Initial Care Plan Setup

Includes initial Eldercare consultation with a responsible party by Zoom or in-person with Eldercare Consultant, Suzanne Hammond and time to transition designated staff person(s) to the new client, set up of client file, service documents, and individualized care plan.

Ongoing service weekly

Visit duration as agreed upon at initial consultation. Minimum of one-hour visits. A minimum of two hour visits for outside outings is desired.
